Medical License

Please upload a clear, complete, and valid copy of your medical license from New Hampshire or another compact state via the link here. Please note that Massachusetts is not a compact state and you must apply for an NH state license by reciprocity. This process can take between 2 and 6 weeks. You must have an active medical license to be able to participate in this course and it is your responsibility to provide this documentation to us before the start of the course. Should you be unable to provide your medical license, you will be unable to participate in the hands-on portion of the course and you will need to pay to register in the next available hands-on practical to become fully certified and receive CME/CEU credit.

Files not named in this manner will be harder to associate with your name and may be returned to you. You will not be permitted to participate in the hands-on portion of the course until we have your active medical license.
